Clinicians and clients explore available options and then collaboratively develop a cohesive end-of-life plan. Clinicians can then facilitate family discussions about the client’s end-of-life plan. Issues addressed in counseling include: choosing a health care proxy, types of medical treatment, pain management directives, funeral/memorial planning and legacy goals.
